Priority violations:

• Demonstration of knowledge of food safety evident throughout the facility or by appropriate responses and actions.

• Handwashing sink is falling off the wall and not draining. Repeat violation. Seven days to repair.

• Items in the steam table are not at 135°F. Items not reheated to 165°F or above prior to placing in hot holding. Ensure water in the steam wells is hot prior to putting food on steam table. Repeat violation. Items reheated as needed.

• Prep table is not working. They are using ice to keep foods cold. Ice not at or above the level of the food. Ice only works properly when the level of the ice is correct.

Nonpriority violations:

• Uncovered food in the walk-in is stored on/below shelves that are rusty and dirty. Repeat violation. Correct immediately by keeping foods covered completely. Replace/recoat shelves as needed.

• Shelves in walk-in are rusty and no longer in good condition. Non food-contact surfaces of equipment that are exposed to splash, spillage or other food soiling or that require frequent cleaning shall be constructed of a corrosion resistant, nonabsorbent and smooth material.

• Loose tiles and grout missing in the kitchen area.

Domino's Pizza, 2075 East Main Street, Suite A

Priority violations:

• Demonstration of knowledge of food safety evident throughout the facility or by appropriate responses and actions.

• Employee did not wash hands prior to food preparation after task change. Corrected.

• No sanitizer concentration in the three-compartment sink. Check sanitizer every time to ensure the concentration is correct.

Nonpriority violations:

• No paper towels at the handwashing sink. Corrected.

• Permit not posted in customer view.

Pizza Hut, 391 East U.S. Highway 412

Priority violations:

• Employee did not wash hands between handling dirty dishes and handling clean dishes. Corrected.

• Handwashing sink is not draining at all. Establishment is using the two-compartment sink in the dish area as a temporary alternative. Soap and paper towels must be available at the temporary hand station (correct today). Establishment has 10 days to repair the handwashing sink. A handwashing sink shall be maintained so that it is accessible at all times for employee use and do not use for purposes other than handwashing.

• Provide an adequate supply of hand cleanser (liquid, powder or bar) at each handwashing sink.

• Provide a supply of individual disposable towels at each handwashing sink in food preparation area.

• Quaternary Ammonia sanitizer is not strong enough. Not showing any concentration on the test strip.

Nonpriority violations:

• Interior of the walk-in door is rusty and no longer smooth and easily cleanable.

• Clean the outside storage shed and remove unnecessary items. Clean behind building and remove unnecessary items.

Harp's, 201 Highway 412 West

Nonpriority violations:

• Deli meats stored above the load line in the bunker cooler. Corrected.

• Clean behind the storage bins in the back room. Possible mice droppings. Carefully clean to ensure any issue has been corrected. Check for new evidence and contact pest control operator.

• Wall covering in meat cutting room is coming away from the wall all around the room. Correct.

• Milk spilled under shelving unit and in the area of returns. Maintain the floor in clean condition.

• Two different condensation leaks in the big walk-in freezer. Do not store food items under leaks. Repeat violation. Repair.

Harps Deli/Bakery, 201 Highway 412 West

Priority violations:

• Containers of Onion Dip and Greek Yogurt Dip were bloated and may have gotten temperature abused. One container exploded when the lid was removed. Items destroyed as needed.

• Carton of raw eggs stored above ready-to-eat foods in the upright cooler. Corrected.

• Chicken tenders were not at 135°F or above. Corrected, reheated to above 165°F.

• Portioned chicken pieces in the basket grab-and-go case in the deli area are not holding at 41°F or below. Items in the back of the basket were in temperature but not the ones in the front.

• Container of chicken salad, gallon of milk and flat of shell eggs exceeded the manufacturers use by date. One package of sliced deli meat in deli case exceeded the use by date.

• Employee was going to use degreaser to clean the cutting board without further cleaning. Degreaser should not be used on food contact surfaces. Use all chemicals according to manufacturer's instructions and purpose of use.

Hawg City Grill, 1004 South Mount Olive, Suite H

Priority violation:

• Items not date marked as needed.

Non priority violations:

• Clean exhaust vents and filters. (Person in charge said they will be cleaned Sunday.)

• Condensation leak in walk-in freezer. Repeat violation.

• Remove unnecessary/broken items from back store room.

• No light in walk-in freezer. The light intensity shall be: At least 108 lux (10 foot candles) at a distance of 30 inches above the floor, in walk-in refrigeration units.

Inn At The Springs/Sweetwater Tavern, 201 W. University Street

Priority violations:

• Demonstration of knowledfge of food safety evident throught the facility or by appropriate responses and actions. Person in charge did not know what the concentrations of the sanitizers for the dish machine or three-compartment sink should be.

• Employee did not wash hands prior to putting on gloves after a task change. Corrected.

• No soap at the bar handwashing sink.

• No paper towels at the bar handwashing sink.

• Sanitizer out at dishwasher. Person in charge did not know how to test the sanitizer concentration. Chlorine should be 50-100ppm. Establishment will wash manually until situation is corrected.

• Sanitizer is not strong enough at the three-compartment sink. Establishment will manually mix sanitizer until repaired. Quaternary Ammonia should be 200ppm.

• Foods not being cooled properly. Some items in refrigerator were above 71°F. Cool foods from 135°F to 70°F or less in 2 hours or less. From 70°F to 41°F or less in 4 hours or less.

• Some foods in two-door cooler were not at 41°F or below. Possibly due to hot foods being placed in refrigerator causing other foods to go out of temperature.

• Items pulled out of the freezer should be remarked with a pulled on or use by date.

• Consumer advisory warning is on the menu but is half cut-off. Items requiring the consumer advisory are not denoted on the menu with an asterish (*).

• Using a cleaning chemical not intended for the use on food contact surfaces on the prep table.

Nonpriority violations:

• Food workers shall wear hair restraints.

• Ice scoop stored on top of ice machine or wine cooler. Store ice scoop on or in something that can be cleaned and sanitized daily. Ice scoop at bar and drink station stored on top of a cloth towel. In use utensils must be stored on something that is non absorbent.

• Counter around the dump sink at the bar is no longer in good condition.

• No cold water handle on the faucet at the three-compartment sink (part on order). Cold water was turned off at the source.

Southside Elementary School, 200 West Tulsa

Priority violations:

• Employee did not wash hands between glove changes. Corrected.

• Employee mixing a very large batch of salad and arms were coming in contact with lettuce. Use utensils or some other way to mix the salad.

Nonpriority violations:

• Large salad bowl was setting on top of a trash can during preparation. Employees were using it to stabilize the bowl so it did not move around. Trash can was empty with a new trash can liner. Corrected.

• Condensation leak in freezer #1. Repeat violation. Small amount of ice build-up on the door and frame of freezer #1.

Feb. 3

Hardee's, 1625 Highway 412 West

Priority violations:

• Employee did not wash hands prior to putting on gloves. Employee did not wash hands after coming in from outside prior to putting packaged food times away.

• Sausage in walk-in still 90°F after being in hot holding for breakfast. Cool foods properly within the approved time frame.

• Lettuce and Blu Cheese were 63°F in the reach-in cooler. Employee said these items were setting out for a while. Corrected.

Nonpriority violations:

• Wall in walk-in cooler is starting to degrade exposing the insulation behind the wall. Insulation is starting to flake and fall onto the floor. Repair properly to meet the regulations. Non food-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent and smooth material.

• Cabinet under the drink station/trash area in lobby is no longer in good condition and no longer easily cleanable. Repair as needed. Non food-contact surfaces of equipment that are exposed to splash, spillage or other food soiling or that require frequent cleaning shall be constructed of corrosion-resistant, nonabsorbent and smooth material.

• Lights in food preparation and store room area are not shielded and no documentation they are shatter-resistant. Repeat violation.

Priority violations:

• Beef on steam table was not at 135°F or above. Reheated. Rice under table was not at 135°F or above. Maintain rice at 135°F or above at all times or cool properly.

• Chili Relleno in prep table was 43°F and guacamole was 42°F. (All other food checked was 41°F or below.)

Nonpriority violation:

• Floor tiles in the kitchen are missing grout.

Siloam Springs Northside Elementary, 501 West Elgin

Nonpriority violation:

• Condensation leak in the small walk-in freezer. Repeat violation.
