• Tortilla warmer was stored directly on top of french toast slices.

• Effective hair restraints are required for food employees. (Hats on order.)

• Some utensils were placed directly on a towel and not in the drain rack. Do not place items directly on a towel.

Priority violations:

• Small piece of pineapple stuck to blending wand. Clean prior to use.

• Not all items date marked as needed.

Nonpriority violations:

• Bag of onions stored on the floor. Store food items at 6 inches off the floor. Repeat violation.

• Knobs missing from the ice cream machine lids. Tape is not an approved repair method. Replace with an approved replacement part.

• Clean floor and wall areas around the three-compartment sinks and mop sink. Clean wall under counter at register.

• Some tiles broken in dish room area. Replace/repair as needed.

Soup and Salad Buffet, 315 W. Tulsa Street

Priority violations:

• Containers stacked inside other containers of uncovered or unprotected food.

• Sanitizer concentration is not strong enough. Should be 50-100 ppm chlorine.

• Food storage containers have a greasy accumulation. Clean well with warm soapy water, rinse off soap and then sanitize.

• Mixed greens are not being held at 41°F or below. When using ice to maintain food cold the ice must completely surround the food item.

• Items not date marked as needed. Repeat violation.

Nonpriority violations:

• Colander is cracked and no longer in good condition. Replace. Repeat violation.

• Lid for customer self-serve ice bin is cracked. Replace. Repeat violation.

• Restroom doors are not self-closing. Adjust hinges as needed.

• Men's restroom has a strong urine smell. Ensure tiles and area around toilet are clean and in good repair.

• Permit not posted in customer view. Repeat violation.
