Priority violations:

• Employees did not lather long enough as required prior to rinsing soap off. Lather for 10-15 seconds then rinse the soap off. Repeat violation.

• Clean the interior and exterior of the ice machine. Parts of the interior of ice machine are rusty and no longer in good condition. The exterior of the ice machine has an accumulation of debris on the top of the ice bin. Repeat violation.

• Bottom of hot holding unit is not holding food at 135°F or above. Unit is not working properly or at all. Do not hold food in the bottom of the unit until repaired.

• Prep table is not holding food at 41°F or below. Repeat violation.

• Items not date marked as needed. Repeat violation.

Nonpriority violations:

• Ice scoop is buried in the ice machine. Store scoop on or in something that can be cleaned and sanitized regularly.

• Shelves in the prep table are rusty and no longer in good condition or easily cleanable. Repeat violation.

• Cove bases in kitchen is coming unglued. Repeat violation that was supposed to be corrected by 12/31/15. New construction.

• Restrooms: Wall behind the sink in the ladies' room is no longer smooth and easily cleanable. Wall around the sink area in the men's room is no longer smooth and easily cleanable. Repeat violation.

• Tiles broken and cracked in customer area. Repeat violation.

• Post the permit in a location in the retail food establishment that is conspicuous to consumers. Repeat.

Nonpriority violations:

• Condensation is leaking onto the cover on the onion ring rack. Do not store food items under leak.

• Employee used cup not scoop to get ice.

• Accumulation of grease around fryers, including the vent in the back of the fryers.

• Grease collected on the exhaust grate and under the panel of the side of ice cream machine.

• Leak in drain of back handwashing sink.

• Tiles broken by the back door.

• Condensation leak in the walk-in cooler.

Priority violations:

• Employee did not wash hands between glove changes.

• White sauce was not cooled properly prior to putting in the walk-in. Corrected by putting in ice bath. Cooked potentially hazardous food (Time/Temperature control for safety food) shall be cooled: (1) Within 2 hours from 57°C (135°F) to 21°C (70°F) p and (2) Within 6 hours from 57°C (135°F) to 5°C (41°F) or less.

• Spinach dip was 113°F in hot holding. Reheated as needed.

• Most foods checked in all the front prep tables are not being held at 41°F or below. Repeat violation. A rick control plan must be implemented to correct this situation permanently.

Nonpriority violations:

• Prep tables are not keeping food at 41°F or below. Repeat violation. Equipment for cooling and heating food and holding cold and hot food, shall be sufficient in number and capacity to provide food temperature in accordance with rules and regulations.

• Tongs stored on oven door handles.

• Permit posted but expired.

Priority violation:

• Chicken salad was not at 41°F or below.

Nonpriority violation:

• Shelf above the coffee grinders is no longer in good condition. Replace as needed. Finish on shelf where grinders are on is coming off. Refinish.
