Harps Deli, 201 Highway 412 West

Priority violations: No paper towels at handwashing sink in deli. Corrected. Employees not washing hands properly or long enough. Cheese display-case thermometer reading 50° F. and 70° F. Product is 45°F.-55°F.

Nonpriority violations: Employee did not wash hands between task change and putting on clean gloves. Sanitizer is back-flowing back into the chemical container.

Pizza Hut, 391 Highway 412

Priority violations: Handle on walk-in door is still not replaced/repaired. Surface is not smooth anymore and actually a hazard to the employees as well as not easily cleanable. Slicer/dicer has not been cleaned of food debris.

Nonpriority violations: Leak under handwashing sink. Leak under three-compartment sink. Strong drain smell in back area by dish machine. Check to ensure floor drains are not dry and sewer gas is coming back up. Ice build-up on floor in freezer still. Dish area around and under sink and back wall could use more cleaning. Back wall of prep table by walk-in is dirty still.

Sweetwater Tavern, 201 W. University

Priority violations: Employee drying hands on cloth towel, not single use towel. Corrected. Employee used mop sink as a handwashing sink. Mop sink can NEVER be used for anything but mop water. Corrected. Using the steam table to reheat items. Items did not reheat to 165°F. in two hours or less. Corrected. Foods in the upright unit are not being held at 41°F. or below. Food destroyed as needed.

Nonpriority violations: Clean dishes stored on towels. Effective beard restraints required for food preparation staff.

Sept. 9

Thai Cafe Asian Cuisine, 808 S. Mt. Olive

Priority violation: Clean saucepans frequently.

Nonpriority violations: Clean exhaust vent and filters. Clean stove, drip pans and fryer. Ice scoop stored inside ice bin laying in ice. Corrected. Ice bags are a single use item and cannot be reused for food storage. Clean wall behind sauce cart.

Southside Elementary, 200 W. Tulsa

Priority violation: Dishwasher is not reaching proper wash or sanitize temperatures. Establishment will manually sanitize until repaired. Was working by the end of inspection. Monitor frequently.

Nonpriority violation: Condensation leak in the freezer. Clean walk-in condenser unit and fan.

Siloam Springs High School, 700 N. Progress Ave.

No violations.

Hampton Inn, 2171 Ravenwood Plaza

Priority violations: Hot food is not being held at 135°F. or above. Food being store above the load line in hot well. Food double-panned so hot water is not in contact with the pan the food is in. Cream cheese being held at room temperature -- package says to keep refrigerated. Product 62° F. Time as a control not being used. Thawing steak and fajita mix on top of the oven. Thaw in refrigerator or microwave as part of the reheating process.

Nonpriority violation: Reusing single use containers to store coffee.

Sept. 10

MiChild Enrichment Center, 920 S. Carl

Priority violations: Spray bottles not labeled with contents. Bleach water in spray bottles in kitchen are not strong enough. Should be 50-100ppm. Bleach bottle not labeled with original label.

Nonpriority violation: Employees' hair restraints are not effective.

Caterpillar Clubhouse No. 2, 920 S. Carl

Priority violations: Items in refrigerator are not date marked. Sanitizer in spray bottle is too strong, over 200ppm. Should be 50-100ppm.

Nonpriority violations: Some utensils in one of the drawers area no longer in good condition. Some rusty and some melted and others rusty. Remove unnecessary, unused utensils or those no longer in good condition. Employee wearing jewelry on hands.

Sept. 12

Siloam Springs Middle School, 600 S. Dogwood

Nonpriority violations: Threshold missing a screw, causing the threshold to be loose. Secure and re-caulk as needed. Condensation leak in freezer and excessive buildup of ice/frost along top of freezer wall in corner.
