Hawaiian Shaved Ice, 803 E. Main St.

Priority violations:

• Clean and sanitize fresh water tank every 14 days.

Non priority violations:

• Shelf must be smooth and easily cleanable (next season).

• Utensils no longer in good condition. Replace ice pick as needed.

• Water hose must be water potable to fill tank (fresh water tank).

• Permit no longer valid with ownership change. New owner has 10 days to process permit application.

Aug. 1

Dollar tree, 1443 Highway 412 West

Non priority violations:

• Evidence of possible mice. Contact pest control operator (submitted request last week).

• Repair holes in area by restrooms and east wall of store room.

• Change mop water and hang mops to dry.

Aug. 2

Holiday Inn Express & Suites, 2100 Highway 412 West

Priority violations:

* Top part of the hot-holding unit is not holding food at 135°F or above. Food destroyed and unit adjusted. Thermometer was reading below 100°F.

• Gravy in the back kitchen refrigerator is not date marked.

Non priority violations:

• Box of apples stored on the floor in kitchen. Cases of dry goods stored on the floor in the store room.

• Single use utensils are stored with mouth contact surface up and out of holder. Single-use utensils must be stored handle up to prevent contamination bof mouth contact surfaces by customers .

• Interior of drawers in kitchen cabinet are not sealed as required, allowing water to be absorbed by the wood. This was a violation noted during the consturction walk-through and was to be corrected prior to the facility opening. This must be corrected using proper materials within 30 days.

• Plastic storage container is pitted and bubbling and no longer in good condition. Destroy as needed.

• Door frame falling off between dining area and food service area.

• Permit not posted in customer view as required.

Aug. 3

The Rib Crib, 3475 Highway 412 East

Priority violations:

• Uncovered beverage glass on top of the hot-holding case. Repeat violation.

• Carton of raw eggs (employee's) stored on top of a sealed box of cheese. Repeat violation. Corrected.

• Automatic dishwasher is not dispensing sanitizer. Bottle is emply. Corrected.

• Queso, gravy, open chicken breast, pan of beans and bag of queso are not at 135°F or above. Queso and gravy on the line are double panned. Beans and bag of queso may not have been reheated properly. Reheat food to 165°F (unless otherwise noted by manufactuerer) before placing in hot-holding. Items reheated or destroyed as needed. Repeat violation.

• Some items in the top part of the salad cooler were 42°F. Monitor salad prep table to ensure it is wroking properly.

Non priority violations:

• Food workers shall wear beard restraints.

Eskimo Friend Tropical Sno, 101 E. Alpine St.

No violations observed at time of inspection.

Aug. 10

Aldi's, 2802 Highway 412 East

No violations were observed at time of inspection.

Little Caesar's Pizza, 200 N. Progress Ave., Suite 15

Priority violations:

• Handwashing sink in the back preparation/warewashing area is shut off. Repeat violation and was supposed to be corrected by 2/9/17. Situation must be corrected by 8/25/17. Contact the Department of Health when corrected.

• Sanitizer dispensed is not strong enough. Must be 220 ppm. Manually mix sanitizer until repaired.

Non priority violations:

• The premises shall be maintained free of insects and controlled to eliminate their presence on the premises. Flies around the grease trap and dish area. Clean area to help control flies.

• Cutting board at sheeting station is not clean. It is no longer in good condition and should not be used. Item moved out of the area.

• Caulk along the back of the three-compartment sink is no longer in good condition and no longer cleanable.

• Three-compartment sink with sanitizer is not clean. Greasy build-up on sides and bottom of sink. Clean sinks prior to using.

• Gaskets on prep table doors have an accumulation of food and possibly mold on them. Clean regularly to prevent the accumulation of dirt and debris.

• Pass-through doors in walk-in have an accumulation of food and possibly mold along the handles, doors and frame. Doors do not stay closed which is allowing for condensation to build up.

• Roll racks used for pans of raw dough have an accumulation of food and what looks like mold on the brackets that hold the pans. Clean regularly.

• Cutting board in sheeting area is not clean and no longer in good condition. Do not use. Item moved out of sheeting area.

• Grease trap lid is missing the plug, allowing gas and smells from the grease trap to escape when sinks are drained.

• Pass-through doors in walk-in do not stay closed. Repair as needed. Repeat violation.

• Permit posted but expired.

Zaxby's, 190 Progress Ave.

Priority violations:

• A few dishes put away as clean but still had some food left behind. Corrected, rewashed.

Non priority violations:

• Employee not wearing beard restraint as required.

• Frozen milk container at shake station has a corner broken off. Repeat violation from 2/14/17.

• Clean interior of the cabinets at customer drink station.

• Seam in flooring by handwashing sink is starting to split. Repair as soon as possible. Water is getting under the flooring, allowing stagnate standing water to collect under the floor surface. Repeat violation from 2/12/16 and 2/14/16. Facility was given 30 days at that time of inspection to correct.
